    9. Deep cervical chain - deep to the sternomastoid and often inaccessible to examination. Hook your thumb and fingers around either side of the sternomastoid muscle

Outer Structures of Oral Cavity
  1. Assesses for malocclusion.
  2. Palpates the temporal mandibular joint.

Mouth
  1. Uses penlight or otoscope light and tongue blade, gauze and gloves as appropriate.
  2. Uses tongue blade to inspect all structures of mouth; lips, buccal mucosa, hard and soft palpates, teeth.
  3. Examines the dorsal, lateral and ventral tongue surfaces.
  4. Palpates suspicious areas for induration and tenderness.
 
Pharynx
  1. Asks person to extend the tongue and say "ah".
  2. Inspects all structures of pharynx: oropharynx, posterior tongue, uvula.
  3. Uses tongue blade if person has difficulty holding tongue flat.
 
Neck
  1. Palpates the lymph nodes of the head and neck.
  2. Asks person to flex neck slightly forward. Uses pads of index and middle fingers to move skin over underlying tissues in each area.
  3. Inspects the area of the anterior neck containing the thyroid gland.
    1. Note deviations and bulges in a relaxed position.
    2. Observes as person extends neck slightly and swallows.
  4. Palpates the thyroid (anterior approach).
    1. Stands facing the person.
    2. Asks patient to extend neck slightly and to swallow.
    3. Uses pads of fingers to feel below the cricoid cartilage for the thyroid isthmus.
    4. Asks the person to swallow and feels the isthmus rising upward under fingers.
    5. Asks person to flex neck slightly forward and to their right.
    6. Places right thumb on the lower portion of thyroid cartilage and displaces it to the person's right.
    7. Hooks the tips of the second and third fingers of the left hand behind the sternomastoid muscle and feels in front of muscle with left thumb.
    8. Palpates for lateral lobe as person swallows.
    9. Reverses the procedure for the left lobe.
    10. Auscultates each lobe for bruits if enlargement noted.
    OR
  5. Palpates the thyroid (posterior approach).
    1. Stands facing the person.
    2. Asks patient to extend neck slightly and to swallow.
    3. Uses pads of fingers to feel below the cricoid cartilage for the thyroid isthmus.
    4. Asks the person to swallow and feels the isthmus rising upward under fingers.
    5. Stands behind the person.
    6. Rests thumbs on back of person's neck.
    7. Uses index and middle fingers of both hands to feel for the thyroid isthmus and for anterior surfaces of lateral lobes.
    8. Asks person to flex neck slightly forward and to the right.
    9. Displaces the thyroid cartilage to the right with fingers of left hand.
    10. Palpates the right lobe using fingers of right hand while person swallows.
    11. Reverses the procedure for the other side.
    12. Auscultates each lobe for bruits if enlargement noted.
